Narrator: Hello, and welcome to NHPD's Web video on the Natural Health Products Ingredients Database Issue Form. This Web video will go through the process for submitting a request to add, remove or revise information found in the NHP Ingredients Database via the Issue Form.

Narrator: This Web video is not meant to provide complete instructions, but to provide an introduction to the tool; it is recommended to read all associated user guides and guidance documents, and attend other webinars and workshops offered by NHPD.

From Health Canada's home page, the NHPID Issue Form is accessible by the following URL: http://www.healthcanada.gc.ca/nhponline. This brings you to the home page for NHPD's Online Solution.

Description: The user types out the URL mentioned above and then hits Enter. The home page for the Natural Health Products Online Solution is displayed.

Narrator: From here you can select the purple button for "Forms and User Guides." Here you will find access to the NHP Online System Standard Terminology Guide. The terminology guide provides additional information on the terminology and naming used in the NHPID, which is very helpful when making requests to the NHPID.

Narrator: You will also find the accompanying User Guide for the Issue Form.

Narrator: The Issue Form itself is a PDF smart form that is downloadable to your desktop.

Start by clicking on the link to the Issue Form. This will bring you to an HTML version of the form. Please note that the HTML version is not a form. To open the form, click on the PDF icon.

This will open the form in your internet browser.

Narrator: At this point you should save the form to your desktop for easy access.

To save the form to your desktop, click on File and then Save As. Name the form as you desire, close and re-open it from your desktop.

Narrator: Please note that the form requires an internet connection for completion.

You may now begin completing the form.

The Issue Form is a smart form and will change depending on the information entered. All required fields are marked with an asterix.

The top of the form has an English or French option button, clicking one or the other will change the language of the form accordingly.

Narrator: Part 1 includes the Requester information. This information must be completed by the individual making the request in case NHPD needs to contact you about the request.

Narrator: Part 2 is the actual request. The Type of Request is a drop-down list. Select the appropriate request type based on the changes you are proposing to the NHPID. Medicinal Ingredient Classification Request, Non-Medicinal Purpose Request, or Other Modification to the Ingredients Database.

Narrator: When we select Medicinal Ingredient Request, the form will change and require different information. Likewise, if we select Non-Medicinal Ingredient Purpose Request, different information is required.

Narrator: Once the form has been completed, and all mandatory fields are filled, you must click on Finalize. The form will check itself and a message will pop up if any mandatory fields are missing.

Narrator: Once successfully validated, a 7 digit tracking number will be generated at the top of the form and the message "This form has been completed" will appear in a green box on the top of the form. Now that the form has been completed, please email it to ingredient support email account, which is quoted on the top of the Issue Form.

Narrator: Once submitted, an auto reply will come from ingredient support saying that we received your request. Requests take 4 weeks on average, to allow for adequate review time and to fit in to the 2 week update cycle of the NHPID.

An Issue Form may be modified by clicking the Modify button, but if creating a new request, please complete a new Issue Form.
