Principles and Approach Regarding Privacy

In keeping with the Privacy Act, the Application Form is confidential once completed. To ensure that all applicants' personal information is protected, the following principles will apply:

  • Only the minimum number of data elements necessary to determine the diversity in the Pool is requested;
  • Personal information will be maintained in a secure environment. This document describes why any information is being collected, under what authority and how it will be protected; and
  • Access to the Pool database will be restricted to authorized personnel. 

In keeping with these principles, information management for participants' data will be undertaken as follows:

  • Access to the Pool database will be restricted to the Pool Secretariat in the HPFB Office of Consumer and Public Involvement (OCAPI);
  • Only the minimum amount of personal information necessary will be collected and only essential contact information shared with HPFB staff (i.e. restricted to names, e-mail and phone numbers for the purposes of inviting participants to consultations);
  • The Pool Secretariat from OCAPI will receive requests from HPFB staff for participants and search the database for a match to the minimal criteria. The Secretariat will then contact Pool participants to determine their interest and availability, then communicate basic contact information to HPFB staff;
  • A Core Privacy Impact Assessment has been conducted to ensure the proper measures have been taken to consider and ensure the privacy of the information received.

Provision of the information requested on this form is voluntary.

The personal information requested in this report is collected under the authority of the Department of Health Act, the Food and Drugs Act, and the Food and Drugs Regulationsfor the purpose of establishing a pool of individuals whose input will be solicited by HPFB programs on a range of health products and food related consultations.

Failure to provide the personal information requested may affect Health Canada's ability to match Pool participants with HPFB programs seeking individuals for consultations.

Personal information that you provide is protected under the provisions of the Privacy Act. The Privacy Act gives you the right to access and/or to correct your personal information. The collection of personal information is described in Personal Information Bank HP PPU 413.
