Guidance Document for the Sales Information Reporting Regulations and Reporting Form

Foreword

This guidance document for the  Pest Control Products Sales Information Reporting Regulations is intended to provide registrants with assistance on how to comply with the Regulations.

Guidance documents are administrative instruments not having force of law and, as such, allow for flexibility in approach. Alternative approaches to the principles and practices described in this document may be acceptable provided they are adequately supported. Alternate approaches should be discussed in advance with the Pest Management Regulatory Agency (PMRA) to avoid the possible finding that applicable statutory or regulatory requirements have not been met.

This document is intended to be used as a reference to help comply with the Regulations. It is not considered a substitute for the Regulations or a stand alone document.

Summary of Steps for Reporting on Sales

  1. Download Sales Information Reporting (SIR) form from the Pesticides and Pest Management portion of Health Canada website.
  2. Save the SIR form on your computer.
  3. Complete SIR form.
  4. Use the e-Index Builder to create and finalize an e-Index file for the SIR form (PMRA Regulatory Zip (PRZ) file created).
  5. Submit the PRZ file to the PMRA Electronic Pesticide Regulatory System (e-PRS) secure channel, on Compact Disk (CD) or Digital Video Disk (DVD), or by e-mail (please include "PRZ" in the subject line). The SIR form will only be accepted in an electronic format.

1.0 Introduction

Pest control products (pesticides) are regulated in Canada under the federal Pest Control Products Act (PCPA).

Under the PCPA, a pesticide must be registered by the Minister of Health before it can be used in Canada. Health Canada's Pest Management Regulatory Agency (PMRA) administers the PCPA on behalf of the Minister. A pesticide may not be registered or may not continue to be registered unless its health and environmental risks and its value have been determined to be acceptable by the Minister. Under the PCPA (Subsection 8 (5)), registrants are required to report sales information about their pesticides.

The Pest Control Products Sales Information Reporting Regulations require registrants of pesticides to submit an annual report to the Minister, indicating the quantity of their products, including all emergency and conditional registrations, made available for sale to users in each province and territory during the previous calendar year. All products with a PCPA Registration Number are required to be reported.

Pesticide sales information will give Health Canada and Canadians a better understanding of pesticide use in Canada during a given year, as well as trends in pesticide use over a longer period of time.

This guidance document is intended to aid registrants in reporting sales of their pesticides as required under the PCPA. This document provides definitions of terms found in the Regulations, details of how the data will be reported, what data needs to be reported, the use of estimated numbers and the dates associated with the collection and reporting of the sales information. The numbers used in the guidance document correspond to the numbering system on the Sales Information Reporting (SIR) form.

Also discussed, are issues regarding the privacy and confidentiality of the information provided to the PMRA and other requirements outlined in the Regulations. This document is intended to be used as a reference to help comply with the Regulations. It is not considered as a substitute for the Regulations or as a standalone document.

1.1 General Information

Personal Information

Personal information is any recorded information that could identify an individual (e.g., name, home address, age, education, SIN, etc.). The Regulations require the person submitting the report on behalf of the registrant to provide personal information including their name, address, fax number, telephone number and e-mail address. Personal information provided on the SIR form will be protected in accordance with the Privacy Act.

Confidential Business Information

The PMRA must protect confidential business information (CBI) in accordance with section 43(4) of the PCPA. The PMRA will consider the product information registrants provide in the Sales Information Report to be CBI, except for the following information that is already publicly available: registrant information, the registration number of a product, the name of a product and the classification of a product.

Public Disclosure

A report on pesticide sales in Canada will be posted on the Pesticides and Pest Management portion of the Health Canada website. This public document will present the sales information in such a way that registrant CBI will be protected from public disclosure. Information considered to be CBI will not be made available to the public through the public report or reading room.

Private Label Products

Registrants who own private label products need to report on their specific registration numbers. This does not take away the obligation of master copy registrants to report sales made to private label registrants. The PMRA will assess the information it receives for any potential overestimation of the quantity of active ingredient sold. If there are questions related to specific products, the PMRA may contact the registrant for clarification.

Provincial Level Data

Registrants are required to submit provincial level data for several questions on the reporting form. If registrants do not have direct access to provincial level data, they can estimate the provincial breakdown of their sales data. Please refer to the section "Estimating Quantities" in this document for more information.

Form Certification

The reporter of the sales information must sign a certification page as the last step in completing the SIR form. The reporter need only type their name into the text signature box provided. A digital signature can also be used. The name must match the name that appears in Question 4. Please do not print or scan the form to sign it by hand. The form must remain in its original electronic format.

1.2 Definitions

For the purposes of the Regulations, the following definitions apply.

What is a Sale?

A sale refers to the quantity of pesticide made available for sale to users. It is not a monetary (dollar) value. If a product is manufactured and only stored in the registrant's warehouse, this is not considered a sale. A sale includes sales to manufacturers in other countries who formulate finished products that do not have a PCPA Registration Number (in other words, paint, fertilizer-pesticide combination products and treated seeds) and are destined for the Canadian market. While the PMRA would like information on sales for the Canadian market, we understand that some registrants may not be able to provide segregated information. In these cases, the registrant should provide any information available, along with an explanation as to the scope of the information. Text boxes have been provided in the SIR form for the explanations.

What are Net Sales?

Products sold by the registrant may be returned to the registrant if not used or sold. The annual net sale is equal to the total gross quantity sold by the registrant minus the quantity of the product returned to the registrant. It should be reported as a quantity and not in dollars. If product sold in one year is returned to the registrant after the sales information report is submitted, the returned product should be reported in the following year's report.

Who is a User?

A user is an individual, organization or corporation that applies a pest control product or uses a pest control product for the manufacture of treated commercial products. A user does not include an individual, organization or corporation that uses a pest control product (pesticide) for treating seed or for manufacturing another pest control product or products regulated under the Feeds Act or Fertilizers Act.

1.3 Reporting on Sales

How to Report Sales Information

The PMRA requires registrants to report their sales data using a standardized electronic format. This format involves registrants filling in a PDF Smart Form (the SIR form), building an e-Index and submitting the e-Index to the PMRA. Reports can also be sent on CD or DVD, by courier or regular mail, or as an electronic file by e-mail (please include "PRZ" in the subject line). Detailed information pertaining to the use of the e-Index and submission of data can be found on the Electronic Pesticide Regulatory System (e-PRS) page.

When Does Reporting Take Place?

Registrants are required to report their sales for a given calendar year, on or before June 1 of the following year. For example, a report for the 2014 calendar year is required to be submitted on or before June 1, 2015. In the event of a situation that endangers human health or safety or the environment or of making a decision under the PCPA, the Minister can request that any available and relevant sales information be reported within 15 calendar days (an Interim Report).

Who Can Report?

The person completing the form must be an employee or duly authorized officer of the registrant company. Agents will not be able to fill in sales reports, but can submit the finished report on behalf of registrants.

Record Retention

Registrants are required to retain all of the original records and supporting data related to the sales information (for both annual and interim reporting) for 6 years after the day on which the report is submitted. These records must be provided to the Minister on request for verification and auditing purposes.

Audited Sales Information

If the Minister has reasonable grounds to believe that the sales information provided is not accurate or complete, the Minister can require the registrant to submit the sales information in a report prepared by an independent auditor qualified under the laws of a province. The registrant will have 60 days to submit the report to the Minister.

Estimating Quantities

When the registrant reports sales by distributors or quantities of pesticide used by manufacturers in the production of other products, estimates can be used. Estimates are allowed when actual data are not available to the registrant. Estimates must be clearly identified as such in the report. There is a check box for this purpose beside each data entry field where there is an option to provide an estimate.

The estimation method must also be described, and its appropriateness explained. If the registrant checks one of the boxes to indicate that an estimate was used, a text box will appear where the estimation method and its appropriateness can be explained. Registrants will have to provide an estimation method for each question where estimates are reported.

If the text box for describing the estimation method and explaining its appropriateness is not large enough (limit: 2000 characters), the information can be sent to the PMRA as an attachment in the PRZ file or by e-mail. Include in the text box of the SIR form: "Data sent as a separate file".

What Units of Measure Are Used for Reporting?

The registrant must report the quantity of each product sold in the unit of measure identified in the declaration of net quantity on the product label. Only metric units are acceptable. For example, the quantity would be reported in "grams (g)", not "cans of 500 g". In cases where there is no declaration of net quantity in metric units on the product label, registrants can select the appropriate unit from a drop down list of other units.

2.0 Sales Information Reporting Form

The SIR form is a PDF Smart Form and unfolds according to the answers provided. A registrant may not encounter all of the numbers and data entry boxes mentioned in this document.

The most up to date version of the reporting form must be used. Please download and save a new copy of the SIR form each reporting year.

2.1 System Requirements

The SIR form was developed using Adobe Acrobat. As the form is updated each year to incorporate new product information using the latest version of Adobe, registrants should ensure that they have the most recent version of Adobe. It is recommended that registrants use the latest free  Adobe Reader to complete the form. This ensures that the form will function properly.

2.2 Downloading the Sales Information Reporting Form

The Sales Information Reporting (SIR) form must be downloaded from the Pesticides and Pest Management portion of the Health Canada website before it can be used. Save the form on your computer or network. Once downloaded, you may open the file on your computer and start using it to create your sales report. Each form allows for 25 products with sales and any number or products with no sales to be declared. The form does not need to be complete to be saved.

The SIR form contains 3 main subforms that cannot be seen at the same time. They are reached by using the command buttons as described below. The majority of the questions on the form are mandatory. All mandatory fields on a subform must be completed before proceeding to the next subform.

Don't forget to scroll down to the bottom of each subform. There may be more questions that are not visible on your screen. If an attempt is made to proceed to the next subform, any mandatory questions that are not answered will appear in red.

Validate Subform Button

The Validate Subform button, located at the top and bottom of each subform, allows you to identify which mandatory fields have not yet been completed. When you validate the subform, all mandatory fields that have not been completed will appear in red. This will also occur when you attempt to move to a new subform. The Validate Subform button does not submit the form for the registrant.

Next Subform and Previous Subform Buttons

The Next Subform and Previous Subform buttons, located at the top and bottom of each subform, allow you to move between subforms. You may only move between subforms after all mandatory fields in the current subform have been answered.

Add Subform and Delete Subform Buttons

The Add Subform and Delete Subform buttons are located at the top and bottom of each page. When the registrant has completed the tables for one product and would like to provide sales information on another product the Add Subform button will add a new product subform. Registrants can add up to 25 product subforms to the same report. The Delete Subform button will remove the current subform.

Show Products Button

The Show products button is located at the top of each page and will show a pop-up window with a listing of all registration numbers entered on the form (both for zero sales and on detailed subforms).

2.4 Characters that Cannot Be Used on the Form

The following characters cannot be used when filling out the sales reporting form: "<", ">", "&", and the "enter" key.

2.5 Zero Default Value

All fields for declaring quantities default to a value of zero.

2.6 How to Answer the Questions on the Reporting Form

Type of Report

1) Type of report

Select annual report from the drop-down menu if the report covers the calendar year. If the report is submitted to amend a previously submitted report, select amendment. A text box will appear where changes can be described.

In a situation that endangers human health or safety or the environment, or a decision needs to be made under the PCPA, an interim report may be requested. If the interim report is selected, the registrant will need to indicate the time period their report covers by choosing the start and end dates.

Registrant Information

2) Full legal name of registrant

Include the full legal name of the registrant company that appears in PMRA official records. Do not use abbreviations of any sort.

3) Postal address of the registrant

  3. Province or State
  4. Country
  5. Postal Code or Zip Code

The registrant should not consider changes in the postal address shown here to be an official notification of address change. Registrants must still use the official notification process.

Reporter's Information

4) Reporter's information

The report is to be completed by an officer or employee of the registrant who is duly authorized to make the report on the registrant's behalf. The registrant is responsible for the report. Registrants cannot authorize an agent to complete the report on their behalf. However, once completed and certified by the registrant company, agents can submit the file to the PMRA. The reporter is the person who will complete the certification by signing the report. Provide the name of the person completing the report and the title they hold within the registrant company.

5) Business contact information

Include the reporter's business telephone and fax numbers (including area code) and e-mail address.

6) Is the reporter's business address the same as above?

If the postal address of the person completing the report is the same as the postal address of the registrant, select "Yes". If "Yes" is chosen, the form will automatically be filled in with the information from Question 3. If the address is different, select "No" and fill in the address details.

7) Address of person/registrant completing the report

Report Information

8) Calendar year of the report

Choose the year that is being reported from the drop-down menu.

9) Date report was completed

Select from the calendar, the day on which the report was completed.

10) Products with no sales

Please provide the PCPA Registration Number for all products with no sales. This includes technical grade active ingredients and manufacturing concentrates that are only used in-house.

Registrants can add boxes for registration numbers by clicking "Add a no sales registration no." Registrants can delete boxes by clicking on the "Delete no sales registration no." button. No further information is required on these products; however a text box will appear for additional information. This text box can be used for any explanations related to products with no sales.

If the registrant would like to include additional information regarding products with no sales, a text box has been provided.

If a registrant has only products with no sales to report, and has finished entering their registration numbers in Question 10, they can proceed to the "Report Completed subform" by clicking the "Proceed to final page (I have no products with sales.)" button that follows.

If a registrant has sold products during the calendar year and has no more products to report in Question 10, they should click the "Add a Sales Information Subform" button to proceed to the next subform.

Product Information

A sales information subform will need to be filled in for each product with a PCPA Registration Number with sales that a registrant owns. Each product will have a separate subform within the SIR form.

Registrants can include detailed sales data on 25 products per form and any number of products with no sales.

11) Registration number

Enter the PCPA Registration Number of the first product with sales. Do not enter registration numbers of products already reported with no sales in Question 10. At the top of the reporting form is a "Show Products" button which will display the registration numbers of all products entered in the form (including those entered in Question 10: No sales). Two data entry fields (Question 12 and 13) will be automatically filled based on the registration number provided in Question 11. If a product transferred ownership during the calendar year, select reporting company from the list that appears in the box. If Questions 12 and 13 are not automatically filled, please check the registration number entered for accuracy.

12) Product name

This field will be filled automatically based on the PCPA Registration Number entered in Question 11, and will display the company name and product names the PMRA has on record.

13) Product classification

This field will be filled automatically based on the PCPA Registration Number entered in Question 11, and will display the product classification the PMRA has on record. The product classification also appears in the table of products requiring reports that is sent to registrants at the beginning of the year. The product classification determines which questions need to be subsequently answered.

14) Unit of measure used to report this product

From the "Unit" drop-down list, select the metric unit of measure being used to report information on this product. The unit of measure in which the product must be reported appears in the products table that is mailed to registrants with their annual reminder letter, and appears in the declaration of net quantity on the product label. Only metric units are acceptable. For example, the quantity would be reported in "grams (g)", not "cans of 500 g".

In cases where there is no declaration of net quantity in metric units (e.g., devices), registrants should select "other" from the "Unit" drop down list, then select the appropriate unit from the next drop down list that appears. If the required unit does not appear in the next list, select "other" and fill in the appropriate unit in the "Other Unit" box that will subsequently appear.

15) Did this product change ownership during the calendar year of this report?

If the registrant acquired or sold the ownership of this product during the calendar year, select "Yes" in Question 15. If the product has remained under the same ownership for the entire calendar year, select "No". When "Yes" is chosen, the registrant will need to indicate the time frame (duration of the ownership in that year) that their report covers. Select the start and the end of the reporting period from the calendars in Question 15a.

16) This field is blank

Technical Grade Active Ingredients and Manufacturing Concentrates

Four questions on the subform capture the quantity of technical grade active ingredients and manufacturing concentrates that are not used in the production of registered end-use pest control products intended for sale in Canada. The quantity of product sold as an end-use product is captured in Questions 21 to 26. The PMRA will assess the information it receives for any potential overestimation of the quantity of active ingredient sold. If there are questions related to specific products, the PMRA may contact the registrant for clarification.

If registered technical grade active ingredients and manufacturing concentrates are sold to manufacturers outside of Canada with the intent that the pesticide will be reformulated into a product for the Canadian market, sales must be reported.

17) Quantity of pesticide sold in Canada (total quantity)

Provide the total quantity of pesticide sold to all manufacturers, users, distributors and vendors in Canada.

18) Is this pesticide registered only for the manufacture of pest control products?

If the pesticide is registered solely for the manufacture of products regulated under the Pest Control Products Act, select "Yes" in Question 18. When "Yes" is chosen no other questions need to be answered and the Additional Information field (Question 27) will appear. If "No" is selected the registrant must provide the total quantity of pesticide sold directly to manufacturers of products regulated under the Feeds Act or the Fertilizers Act in Question 18a. The quantity of product intended for use in the manufacture of fertilizer-pesticide and feed-pesticide combination products, and intended for the Canadian market, should be included, if possible. If the manufacturer also makes products regulated under the Pest Control Products Act, the quantity of pesticide they use to make these products should be subtracted from the total sold to the manufacturer.

If the total quantity of pesticide used to manufacture fertilizer-pesticide and feed-pesticide combination products intended for the Canadian market cannot be separated, registrants are asked to provide the quantity sold to the manufacturer in Question18a and an explanation of this quantity in Question18b. This may include information on the range of products manufactured by the buyer, and whether it is known if all original product sold will be used to manufacture products destined for the Canadian market. As an example, a registrant may sell a pesticide to an American manufacturer who makes many different types of products and only some of that pesticide is manufactured into products destined for the Canadian market. The registrant should explain in as much detail as possible, how the quantity declared in Question 18a represents only what is sold into Canada.

19) Can the quantity of pesticide contained in the products made by the manufacturers in Question 18a, and made available for sale by vendors to users, be reported or estimated by the registrant (province and territory totals)?

If the registrant can report or estimate the quantity of pesticide being made available for sale by the manufacturer in Question 18a, select "Yes" in Question 19. When "Yes" is chosen the registrant will be asked to provide the quantity of pesticide contained in products regulated under the Feeds Act or the Fertilizers Act broken down by province and territory (Question 19b). This is the quantity of the pesticide in the feed-pesticide products and the fertilizer-pesticide products. Estimates can be used. If the quantity provided for a particular province or territory is an estimate, select "Yes" (Question 19c) beside the quantity. The form default is set to "No" estimates used. If "Yes" is selected, a text box will appear in Question 19d. Include the estimation method and an explanation of the appropriateness of the estimate method. (See section on Estimating Quantities for more details.)

20) Quantity of pesticide sold for the manufacture of products without a Pest Control Products Act Registration Number such as paints and plastics (net total quantity, province and territory totals)

Provide the quantity of pesticide sold directly by the registrant to manufacturers, minus the total of any product returned by the manufacturer to the registrant. This could include manufacturers of products such as fungal-resistant paints and caulking. Do not report sales to commercial seed treatment facilities or manufacturers of products with a PCPA Registration Number or products regulated under the Feeds Act or Fertilizers Act (Question 20a).

If the total quantity of pesticide used to treat seed or manufacture products with a PCPA Registration Number or products regulated under the Feeds Act or Fertilizers Act cannot be separated, registrants are asked to provide the quantity sold to the manufacturer (Question 20a) and an explanation of this quantity in Question 20b. This may include information on the range of products manufactured by the buyer, and whether it is known if the entire quantity of original product sold will be used to manufacture products destined for the Canadian market. As an example, a registrant may sell a pesticide to an American manufacturer who makes many different types of products and only some of that pesticide is manufactured into products destined for the Canadian market. The registrant should explain in as much detail as possible, how the quantity declared in Question 20a represents only what is sold into Canada.

At this point, registrants will be able to fill in Question 27 (refer to instructions below). The registrant should verify that they have answered all the questions by clicking the Validate Subform button. Missing information will be highlighted in red when the validate subform button or navigation buttons are clicked. Once completed and validated, the registrant can either proceed to the certification subform by clicking the Next Subform button, or add a new product subform for another product by clicking the Add Subform button.

End-Use Products (Commercial, Restricted, Domestic, and Commercial + Restricted)

Questions 21 to 26 on the subform capture the quantity of end-use products intended for sale in Canada. The quantity of technical grade active ingredients and manufacturing concentrates is captured in the preceding section of the form (Questions 17 to 20). This separation of information ensures that quantities are not being double counted.

21) Quantity of pest control product sold in Canada (total quantity)

Provide the total quantity of pest control product sold to all manufacturers, users, distributors, vendors and seed treatment facilities in Canada.

22) Is this product sold to treat seed or manufacture products regulated under the Feeds Act or Fertilizers Act?

If the registrant makes their product available for sale to commercial seed treatment facilities or manufacturers of products regulated under the Feeds Act or Fertilizers Act, select "Yes" in Question 22. When "Yes" is selected the registrant will be asked to provide the total quantity of pesticide sold directly to the manufacturers of products regulated under the Feeds Act or the Fertilizers Act or to commercial seed treatment facilities (Question 22a). If possible, please include the quantity of pesticide intended for treating seed or manufacturing feed-pesticide or fertilizer-pesticide combination products intended for use in Canada.

If the total quantity of pesticide sold for use in fertilizer-pesticide and feed-pesticide combination products and treated seeds intended for use in Canada cannot be separated, registrants are asked to provide the quantity sold to the manufacturer in Question 22a and an explanation of this quantity in Question 22b. This may include information on the range of products manufactured by the buyer, and whether it is known if all original product sold will be used to manufacture products destined for the Canadian market. As an example, a registrant may sell a pesticide to an American manufacturer who makes many different types of products and only some of that pesticide is manufactured into products destined for the Canadian market. The registrant should explain in as much detail as possible, how the quantity declared in Question 22a represents only what is sold into Canada.

23) Can the quantity of pesticide contained in the products made by the manufacturers in Question 22a, and made available for sale by vendors to users, be reported or estimated by the registrant?

If the registrant can report or estimate the quantity of pesticide being used by the manufacturer in Question 22a, select "Yes" in Question 23. When "Yes" is chosen the registrant will be asked to provide the quantity of pesticide contained in products made by the manufacturers in Question 22a, and made available for sale by vendors to users, broken down by province and territory (Question 23b). This includes combination fertilizer-pesticide products, feed-pesticide products and treated seeds. Estimates can be used. If the quantity provided for a particular province or territory is an estimate, select "Yes" (Question 23c) beside the quantity. The form is set to default to "No" estimates used (Question 23c). If "Yes" is selected, a text box will appear for a description of the estimation method and an explanation of the appropriateness of the method (Question 23d). See section on Estimating Quantities for more details.

24) Quantity of pest control product sold directly to regional and national distributors (net quantity, province and territory totals)

Include the total quantity of pest control product sold directly by the registrant to regional and national distributors, minus any returns by the distributors to the registrant. A regional distributor is a distributor based in one province who distributes their product throughout a region that includes several provinces.

25) Can the quantity of pest control product made available for sale by regional and national distributors be reported or estimated by the registrant (net quantity, province and territory totals)?

If the registrant can report or estimate the quantity of pesticide being made available for sale by regional and national distributors in Question 24, select "Yes" in Question 25. When "Yes" is chosen the registrant will be asked to provide the quantity of pest control product made available for sale to users by regional and national distributors, broken down by province and territory (Question 25b).

If the quantities to be reported for Question 25 are the same as those in Question 24, click the Copy button. The form will automatically be filled in with the data from Question 24. If "No" is selected, provide the quantity of pesticide made available for sale to users by regional and national distributors by province and territory in Question 25b). Estimates can be used. If the quantity provided for a particular province or territory is an estimate, select "Yes" from the drop-down menu in Question 25c, beside the quantity. The form is set to default to "No" estimates used in Question 25c). If "Yes" is selected, a text box (Question 25d) will appear. Include the estimation method and an explanation of the appropriateness of the method. (See section on Estimating Quantities for more details.)

26) Quantity of pest control product sold directly to users, vendors and provincial distributors (net quantity, province and territory totals)

Provide the sum of the quantities sold directly to users, vendors and provincial distributors, minus any returns to the registrant. A provincial distributor is one who distributes products within one province only. Be sure not to include sales made by distributors mentioned in Question 24. In this question, users can include manufacturers who use an end-use pesticide as a material preservative (e.g., in paint) or for treated articles (e.g., fungal-resistant paints).

Additional Information

27) Additional information for this product

Registrants can provide any additional information in this space. Examples of what can be provided include a description of products that do not seem to fit into the categories described in the reporting form or products that are not made available for sale in a conventional manner. Do not include sales data.

The registrant should verify that they have answered all the questions by clicking the Validate Subform button. Missing information will be highlighted in red when the Validate Subform button or navigation buttons are clicked. Once validated, the registrant can either proceed to the certification subform by clicking the Next Subform button, or add a new product subform for another product by clicking the Add Subform button.

Form Certification

After the registrant has filled in data for all their registered products, the certification subform must be dated and signed. This is the last step on the reporting form and is reached by clicking on the Next Subform button at the top or bottom of each page. The registrant must type their text signature into the box provided to verify and certify the completeness and accuracy of the information. A digital signature can also be used. The name must match the name that appears in Question 4. Certification indicates that the information in the report is true and complete to the best of the reporter's knowledge and belief and is provided in good faith.

Once saved as a PDF file, the electronic SIR form can be incorporated into an e-Index by using the e-Index Builder. Registrants can then send in the resulting PRZ file via the Electronic Pesticide Regulatory System (e-PRS) Secure Web Portal, on a CD or DVD by courier or regular mail, or in electronic format by e-mail (please include "PRZ" in the subject line). Please note that submitting this report by e-mail to pmra.docs.arla@hc-sc.gc.ca is not considered a secure means of transmitting confidential business information.
