I am pleased to inform you that the new form entitled "Export Certificate", is now available on the Health Canada's Compliance and Enforcement website at:

http://hc-sc.gc.ca/dhp-mps/compli-conform/gmp-bpf/form/index-eng.php

An Export Certificate is issued in the prescribed form and manner, stating that the package and its contents do not contravene any known requirements of the law of the country to which it is or is about to be consigned for drugs that are fabricated in Canada and sold for consumption outside of Canada when the package is clearly marked "Export" or "Exportation".

The Export Certificate used for Medical Devices can be found in Schedule 3 of the Medical Device Regulations.
