This survey is meant for people who have a high level of knowledge about activities and oversight within their hospital blood bank.

You have until July 6, 2007 to complete the survey. A Consultation Report that includes findings from both the Online Workbook and Online Survey will then be drafted and posted to this website.

Your responses to this survey will help us to better understand Canada's blood system at a hospital and regional level, and will inform the further refinement of proposed regulatory requirements for specific blood related activities.

Please refer to the Glossary (also available in PDF format) to understand how Health Canada is defining the technical terms you find below. Although the survey is short, it need not be completed in one sitting - each section may be completed and submitted independently.
