How to Make Changes to your Existing Authorization and/or Licence to Produce

There are different types of changes that you may make to your existing Authorization to Possess and/or your Licence to Produce (either your licence or the licence of the person you have designated to produce your supply of dried marihuana). Some of these changes require the use of one or more forms, while others require a different procedure, as explained below.

With the implementation of the new Next link will take you to another Web site Marihuana for Medical Purposes Regulations, all authorizations and/or licences will expire on March 31, 2014. After that date marihuana for medical purposes may only be accessed via a Health Canada authorized licensed producer.

As of October 1, 2013, Health Canada will no longer accept applications to increase the number of plants or change addresses noted on Personal-Use Production Licences and/or Designated-Person Production Licences.

Corrections or Lost or Stolen Documents

  • In the case of a correction to an error made to your authorization and/or licence (for example, name or address misspelled), you can send a letter indicating the error to the MMAP to the following address:

  • To declare lost or stolen documents and to request replacement documents, please send a letter to the MMAP indicating the loss and include the police report number.  Mail it to:

Changes to your Authorization to Possess

Name Change

  • Form A
  • Legal name change document or marriage certificate

Change of address (if your source option is Health Canada supply or a Designated Person)

Increase in daily amount

  • Form A - IF your daily amount will be above 5 grams/day
  • Form B1 or Form B2 completed by your supporting physician

**Please note that if you access your supply via a Personal-Use or Designated-Person Production Licence, increases in number of plants must be received by Health Canada before October 1, 2013.

As of October 1, 2013, an interim supply of dried marihuana may be ordered from Health Canada to address the increase in your daily amount until you wish to switch to a licensed producer for your source of supply.

Revoking your Authorization

A letter from you stating that you wish to revoke your Authorization to Possess.

Changes to your Personal-Use Production Licence

Change of Address

If production will be affected by the address change:

  • Form C
  • Form F if the production site is not the applicant's or designated person's ordinary place of residence and is not owned by the applicant or designated person.
    • Proof of signing authority if the property owner named is a Corporation.

**Please note that applications for changes in address that affect the production site must be received by Health Canada before October 1, 2013.

Also, Health Canada will not issue amended production licences for any address changes after December 15, 2013.

Change of production location

  • Form C
  • Form F if the production site is not the applicant's or designated person's ordinary place of residence and is not owned by the applicant or designated person.
    • Proof of signing authority if the property owner named is a Corporation.

Changes to your Designated-Person Production Licence

Change of Designated Person's address

  • Form D
  • Form F if the production site is not the applicant's or designated person's ordinary place of residence and is not owned by the applicant or designated person.
    • Proof of signing authority if the property owner named is a Corporation.

**Please note that changes in address that affect the production site must be received by Health Canada before October 1, 2013.

Also, Health Canada will not issue amended production licences for any address changes after December 15, 2013.

    • Proof of signing authority if the property owner named is a Corporation.

** Please note that applications for new Personal-Use Production Licences must be received by Health Canada before October 1, 2013.

Applications received on or after this date will be returned.

From Health Canada Supply to Designated-Person Production

  • Form A
  • Form D
  • Original CRC of the Designated Person
  • 2 Designated Person photographs at least one signed by AP
  • Form F if the production site is not the applicant's or designated person's ordinary place of residence and is not owned by the applicant or designated person.
    • Proof of signing authority if the property owner named is a Corporation.

** Please note that applications for new Designated-Person Production Licences must be received by Health Canada before October 1, 2013.

Applications received on or after this date will be returned.

From Personal-Use Production to Health Canada Supply

  • Signed written letter revoking PUPL
  • Form A indicating Health Canada Supply as source
  • Form E1 (if no dried supply previously)

From Personal-Use Production to Designated-Person Production

  • Signed written letter revoking PUPL
  • Form A indicating new Designated-Person Production source
  • Form D
  • Original CRC of Designated-Person
  • 2 Designated-Person photographs at least one signed by AP
If applicable:

Applications received on or after this date will be returned.

From Designated-Person Production to Health Canada Supply

  • Signed written letter from Designated-Person revoking Designated-Person Production Licence
  • Form A indicating Health Canada Supply as source
  • Form E1 (if no dried supply previously)

From Designated-Person Production to Personal-Use Production

  • Signed written letter from Designated-Person revoking Designated-Person Production Licence
  • Form A indicating PUPL as source
  • Form C

Changes to your Health Canada Supply

Add Seeds

If you have previously applied for seeds:

If you had previously applied for seeds/interim supply:
  • Letter explaining why more seeds/interim supply are required

The MMAP strives to meet its service standards for processing complete applications within 10 weeks of receiving the application, including new applications, applications to renew, or applications to amend an existing authorization/licence. Please note that the service standard only applies once applications are fully completed. Incomplete applications are returned to the applicant with instructions on what is missing and how to fill in missing information. Applications from people with symptoms being treated within the context of compassionate end-of-life care will continue to be given immediate priority.
