How to Make Changes to your Existing Authorization and/or Licence to Produce
There are different types of changes that you may make to your existing Authorization to Possess and/or your Licence to Produce (either your licence or the licence of the person you have designated to produce your supply of dried marihuana). As dictated by the
Marihuana Medical Access Regulations, some of these changes require the use of one or more forms, while others require a different procedure, as explained below.
Corrections or Lost or Stolen Documents
- In the case of a correction to an error made to your authorization and/or licence (for example, name or address misspelled), you can send a letter indicating the error to the MMAP to the following address:

- To declare lost or stolen documents and to request replacement documents, please send a letter to the MMAP indicating the loss and include the police report number. Mail it to:

Changes to your Authorization to Possess

- Form A
- Legal name change document or marriage certificate

| Change of address (if your source option is Health Canada supply or a Designated Person Person Production) |

- Form A - IF your daily amount will be above 5 grams/day
- Form B1 or Form B2 completed by your supporting physician

| Revoking your Authorization |
A letter from you stating that you wish to revoke your Authorization to Possess. |
Changes to your Personal Use Production Licence
| Change of Address |
If production will be affected by the address change:
- Form C
- Form F if the production site is not the applicant's or designated person's ordinary place of residence and is not owned by the applicant or designated person.
- Proof of signing authority if the property owner named is a Corporation

The MMAP strives to meet its service standards for pprocessing complete applications within 10 weeks of receiving the application, including new applications, applications to renew, or applications to amend an existing authorization/licence. Please note that the service standard only applies once applications are fully completed. Incomplete applications are returned to the applicant with instructions on what is missing and how to fill in missing information. Applications from people with symptoms being treated within the context of compassionate end-of-life care will continue to be given immediate priority.
