Marihuana Medical Access Program
Note: these instructions apply to the Marihuana Medical Access Regulations. If you are looking to access marihuana from a licensed producer under the new Marihuana for Medical Purposes Regulations (MMPR), you do not apply to Health Canada. Please find the relevant instructions here.
We recommend that you read Information for the Patient and How to Apply for Marihuana for Medical Purposes - First Time first and then download the forms that are pertinent to the type of access that you are requesting. You will need to fill out the appropriate applications for Authorization to Possess, Licences to Produce and for supply.
Health Canada's examination of the current available information suggests most individuals use an average daily amount of 1 gram to 3 grams of dried marihuana for medical purposes, whether it is taken orally, or inhaled or a combination of both.
The Canadian Medical Protective Association (CMPA) has developed a "Release Form for Medical Practitioners". This form is available on the Canadian Medical Protective Association website. This is not a Health Canada form and completing the form is not a requirement to obtain an Authorization to Possess marihuana for medical purposes. Any questions relating to this form should be discussed with your physician.

All Personal-Use Production Licences will expire March 31, 2014. On that date, all dried marihuana and plants produced under Personal-Use Production Licences must be destroyed.
