2012 Annual Drug Notification and Fee Remission Request

Please note that in order to process the fee remission request, companies must ensure that the Right to Sell Drugs Fee Remission Request and Attestation Form (the Form) includes the following information:

  • the Annual Gross Revenue from the sale of the drug in the previous calendar year reflecting sales from January 1, 2011 to December 31, 2011; and
  • the Calendar Year End Date entered in the Form is December 31, 2011.

If the information specified above is not included in the Form, the October 2012 invoice may not reflect the reduced fee(s).
