Labelling Standard: Animal Tissue Form

The Animal Tissue: Category IV (1996) and Animal Tissue: Labelling Standard (1995) forms have been replaced by Appendix 4 of the revised Health Canada 3011: Drug Submission Application Form (2010). Please provide this completed form when applying for a drug product compliant with a Category IV Monograph or Labelling Standard. For further information, please refer to the HC 3011 Form.
