Request for Controlled Substances for Drug Detector Training Purposes - Guidance to Completing the Form

Form HC9207 is used to request controlled substances for training purposes, and again, throughout the training kit's history. There are a total of five steps involved in the kit's history.

Step 1 - Request New, Replacement or Addition to Kit

(1) Check the appropriate box

  • New kit: handler has not previously received a kit from Health Canada (i.e., new handler)
  • Replacement kit: handler is replacing an existing kit (kit is 3-5 years old or has become contaminated)
  • Addition to kit: handler requires an additional drug for an existing kit

(2) Complete Section 1 - Kit Requested By

  • Full name: name of person requesting kit. This is the person the DDTK unit will contact if there areany questions concerning the request
  • Rank/Title: rank/title of requester
  • Address: address of requester
  • Telephone and Fax: telephone and fax number of requester
  • Email: email address of requester or unit
  • Signature and Date: signature of requester and date signed

(3) Complete Section 2 - Deliver Kit

  • Institution: full name of police force, institution, agency, kennel, etc.
  • Address: to be completed if address is different from Section 1

(4) Complete Section 3 - Dog Handler Information

  • Handler: full name of dog handler
  • Telephone: telephone number where handler can be reached (number will be used on courier waybill)
  • Rank: rank of handler, if applicable
  • Dog: name of dog detecting drugs

(5) Complete Section 4 - Requirements

  • Handler: tick the box against the controlled substances required for detector training purposes

(6) Complete Section 8 - Authorized By

  • Name & Rank: name and rank of individual authorized to approve training kit requests
  • Signature & Date: signature of authorized individual and date form signed
  • Address: check appropriate box or insert address if different from Section 1 or 2

(7) Mail Request

  • Mail original HC9207 form to the DDTK unit and retain a copy for your files.

Step 2 - DDTK Unit Receives Your Request

Upon receipt of request, the unit will review contents of form. Once approved, we will provide your institution with the drugs requested (if available) and indicate the quantity provided for each controlled substance and its strength/descriptor (if applicable) in Section 5. The bag/container weight and inventory control numbers are for DAS use.

The unit will keep the original form, include a copy with the training kit issued, and mail a copy to the requester (if different from handler) and authorized official.

Step 3 - Institution Receives Training Kit

Upon receipt of the training kit, the handler must verify the quantity of each drug received, indicate if there are any discrepancies and initial under Section 6.

Once verified, we require that the form be faxed to us within two business days of receipt to ensure that requested drugs were received and to record discrepancies, if any.

Important

  • The form should remain with the training kit at all times. When the kit is returned to the DDTK unit, a copy of the form must be included in the return shipment (for auditing purposes).
  • All drugs must be returned in their original zip bag. Along with the drugs, any contaminated materials (i.e., training props) should be returned for destruction.
  • Drugs in the training kit are NOT to be mixed together (i.e., physically mix the contents of cocaine with the contents of heroin, etc.).

Step 4 - Institution Returns Training Kit

The dog handler must complete Section 7 of the form. For each drug issued, the quantity returned and difference between issued and returned amounts needs to be completed, along with the handler's initials.

Any discrepancies (+/- 15%) between the quantity provided by the DDTK unit and the quantity being returned must be documented (in memo or letter form), signed by an authorized official and included in the returned kit.

If any of the drugs issued were contaminated during training (i.e., a drug was contaminated by coming into contact with another drug), a memo or letter, signed by an authorized official, explaining how the drug became contaminated must accompany the returned kit. This information is required for analysis purposes.

Return training kit and HC9207 form via bonded courier to the DDTK unit.

Step 5 - DDTK Unit Receives Used Training Kit

Upon receipt of the used training kit, the unit will verify quantities returned. The used kit is then sealed and stored for destruction by a certified medical waste company.
