Statements of Need for Postgraduate Training in the United States - Medical graduates - List of Acceptable Documents for Proof of Residence in Ontario or British Columbia

The following documents are acceptable as proof of residence in Ontario or British Columbia. All financial information should be blocked out.

• Child Tax Benefit Statement
• Employer Record (pay stub; letter from employer on company letterhead)
• Income Tax Notice of Assessment (most recent)
• Insurance Policy (home; auto; life)
• Monthly mailed bank account statements for savings or chequing accounts only
• Ontario or British Columbia Motor Vehicle Permit (plate or vehicle portions)
• Property tax bill
• Ontario or British Columbia School, college or university report card or transcript
• Statement of Direct Deposit for Ontario Works
• Statement of Direct Deposit for British Columbia income assistance program
• Statement of Employment Insurance Benefits Paid (T4E)
• Statement of Registered Retirement Savings Plan (RRSP) from an Ontario or British Columbia financial institution (bank; trust company; credit union)
• Utility bill (home telephone land line; cable TV; public utilities commission; hydro; gas; water)

The following documents are not acceptable as proof of residence in Ontario and British Columbia.

• A temporary Driver's Licence
• Credit Card Statement
• Mortgage Agreement; Home Lease or Rental Agreement
• Bank receipts, bank books, bank letters, automated teller receipts
• Cell Phone Bill
