Project: A System to Optimise the Planning and Delivery of Primary Health Care

Background

The three problems that hinder optimal primary care are: delays in obtaining needed information for clinical decision making; delays in applying relevant research to practice; and lack of information for evaluation and planning of health services at the population level.

Project Description

The project will integrate several existing initiatives into an innovative service delivery and planning model for primary health care delivery. It will address delays in obtaining information for clinical decision making, delays in applying relevant research to practice and lack of information for evaluation and planning of health services at the population level. The proposed model will have five key elements: 1. an electronic transmission of dispensed prescriptions and lab results to primary care physicians; 2. a dynamic electronic consent interface; 3. an automated patient level alerts for problems; 4. an infrastructure to electronically update standards for optimal care; and 5. a population health monitoring system.

Project Funding under HISP: $500,000.00
