Project: Tele-Home Care: An Integrated Transition Between Hospital and Home

Background

A discrepancy has emerged in the complexity and intensity of care that is provided for patients at home versus the hospital. While home care services have increased in Canada, services are still provided almost exclusively for individuals who require care easily delivered at home. These changes in health care practices have focused attention on the need for services for patients with intermediate care needs. Patients with these needs are concerned because frequently they do not meet the typical criteria for sustained hospitalization, or community care services at home.

Project Description

The project will design, implement, evaluate and disseminate the results of a new method of home care service delivery. The home care service will be provided through a specialized telecommunications network to families with children, who have acute monitoring needs. The monitoring station in the hospital would permit 24 hour access to speciality consulting. The project will address both technology applications and service integration issues.
