The HTML versions of the documents are not forms; they display the information as found on the forms for viewing purposes only and will not be accepted if used to apply with the Research Ethics Board.
If you wish to submit a form, you must use the PDF version, complete the form and mail or fax it to the Research Ethics Board. These PDF forms are also fillable on your computer.

To view, complete, and print the PDF fillable forms, or to download blank forms, you need Adobe Acrobat Reader version 4.05 or higher. This software can be obtained free from the
Adobe Web site.
In order to save a completed form that contains your own data, you must have Adobe Acrobat Professional 5.0 (full version) or higher. If you do not have this software, you may
purchase Adobe Acrobat Professional online.
